NOT A PATIENT?
No problem! At checkout, choose the Chicago Dental Studio location most convienent for you and we will get you in for a 3D scan so you can start your subscripton.

Can I sign up if I'm not a patient?
Yes! Simply sign up and choose your preferred Chicago Dental Studio location to recieve your 3D scan and we will coordinate with you to get you scheduled for your digital scan!
When will my subscription order be shipped?
Your subscription will be sent on the same date every 6 months/1 year – depending on the frequency of the CDS subscription plan you chose.
Can I change my payment method?
Yes! Just log on to your subscription portal and make any changes to your payment method you need to!
How do I change my delivery address?
You can do so by logging on to your subscription portal and updateing your personal detials.
Can I cancel my subscription?
Yes, you can cancel your subscription at any time once your 2-cycle minimum has been completed by logging into your subscription portal or by submitting a cancellation request using the contact us form found on this page. However, please note that if it is within 10 days of your next billing date it is too late to cancel your next order. Therefore we will bill and ship your next order but every subsequent order after that will be cancelled.
Can I pause my subscription?
Yes, you can pause your subscription via your subscription portal. Please note: if it is within 48 hours of your next billing date it is too late to pause your next order. Therefore, we will bill and ship your next order but every subsequent order after that will be paused.
Can I schedule the delivery?
No, it is not possible to schedule your delivery. Your order will be shipped on the calendar date that you originally ordered and delivery time may vary month on month.
